Indications

When should you see a neurosurgeon?

A neurosurgeon consultation may be needed if:

  • Back or neck pain lasts a long time or spreads to an arm or leg.
  • An MRI has shown a herniated disc or narrowing of the spinal canal.
  • You notice numbness, tingling or reduced sensation in the limbs.
  • You have numbness in the little and ring fingers, especially when bending the elbow.
  • Your hand feels weak and it becomes hard to hold objects.
  • Previously prescribed treatment has not given the expected result.
  • You need an expert opinion on whether neurosurgery is appropriate.

Appointment

What happens at the consultation?

During the visit the doctor reviews the patient’s complaints, medical history and the results of previous tests.

  1. 1

Conversation and review of test results

Dmitry Gavrilin reviews the patient's complaints, medical history and the results of previous tests, including MRI, CT and electroneuromyography (if available).

  2. 2

    Assessing the causes of symptoms

Based on the clinical picture, the doctor assesses the possible causes of the symptoms and decides whether additional tests are needed.

  3. 3

    Treatment plan

The doctor discusses the next steps: what can be done conservatively and whether there are indications for surgery.

Not every patient needs surgery. One of the key aims of the consultation is to determine whether conservative treatment is possible or whether there are indications for surgery.

You can also ask for a second opinion from a neurosurgeon if surgery has already been recommended and you want to clarify the indications, possible treatment options and next steps.

Clinical practice

Experience with complex neurosurgical cases

In May 2026, Dmitry Gavrilin took part in the work of the multidisciplinary team at Riga East Clinical University Hospital that successfully performed a complex neurosurgical procedure on a patient with an extremely rare spinal cord pathology.

The condition involved entrapment of the spinal cord in a defect of its membrane and progressive neurological impairment. Only a few hundred similar cases have been described in the medical literature.

While preparing the treatment, the specialists studied international clinical experience and consulted foreign experts. The operation was performed using microsurgical techniques and intraoperative neuromonitoring.

This clinical case reflects the doctor's experience in diagnosing and treating complex spinal cord pathologies.
